Information, not advice

ClaraCover provides general information about laboratory test prices and Medicare coverage rules. It is not medical, billing, legal, or insurance advice, and it is not a coverage determination. Coverage decisions are made by Medicare when a claim is processed. Talk to your doctor about what tests you need.

Accuracy and dates

Every page shows the date its information was last verified and cites its sources (Medicare fee schedules and coverage policies, and lab consumer prices). Prices and policies change; always confirm current amounts with the lab or Medicare before making decisions.

Concierge answers

If you send us codes through the concierge form, a person looks up the published rules and prices and emails you what they say. That answer is the same kind of general information as the rest of this site. It is not a promise that Medicare will pay, and it does not replace asking the lab or Medicare. What you do with it is your decision.

The lookup tool

The lookup tool shows what published Medicare fee schedules and coverage policies say about the codes you enter. It is the same kind of general information as the rest of this site, and the same limits apply: it is not a coverage determination and not a promise that Medicare will pay. We limit how many lookups a browser may run (a small number with no sign-up, and a daily number after you confirm an email address) to keep the tool available to people rather than programs; we may change those numbers. Automated or bulk use of the tool is not permitted. Voluntary contributions do not create a customer relationship. The concierge service remains available if you need more.

The Medicare cost comparison

The cost comparison at /medicare-cost-comparison/ is arithmetic run in your own browser: it applies Medicare's published, standardized Medigap benefit rules to the amounts you enter, and its premium figures — and the entire Medicare Advantage line, whose copays are inputs — are estimates unless you replace them with real quotes and real plan documents. It is general information, not insurance, financial, or investment advice, and not a recommendation to buy, keep, or drop any plan. We are not an insurance agent or broker and sell no plans. Numbers you enter and files you open with it stay on your device and are not sent to us, with one labeled exception: choosing "Email me my summary" sends us your address and the on-screen totals so we can email them to you, as the Privacy Policy describes.

The claims review

The claims review described at /claims-review/ is a paid service: a person reads the Medicare records you choose to share and writes down what each claim means under the published rules, what was owed and why, and a worksheet for comparing a plan's stated costs. It is explanation and arithmetic. It is not medical, legal, insurance, or financial advice; we are not an insurance agent or a licensed advisor, we do not recommend or sell plans, and we do not contact providers, plans, or Medicare for you. The fee is stated on the page, is invoiced after an initial call, and is refunded in full if you tell us within seven days of delivery that the review was not useful.

No warranty

ClaraCover is free and is provided “as is” and “as available.” We work hard to be accurate, but we make no warranties of any kind, express or implied, including that the information is complete, current, error-free, or fit for a particular purpose.

Limit of liability

To the fullest extent the law allows, DFM USA LLC and its owners are not liable for any indirect, incidental, special, or consequential damages, or for any medical bill, lab charge, or denied claim, arising from your use of this site or of a concierge answer. Because the service is free, our total liability for any claim is limited to $100.

Governing law and changes

These terms are governed by the laws of the State of Indiana, and any dispute will be handled in the state or federal courts located in Indiana. We may update these terms as the service grows; the date at the top of this page shows the latest version.
